Who Can Refer?
Referrals are accepted from a variety of health professionals, including:
- Hospital doctors and nurses
- GPs and community nurses
- Specialist nurses
- Nursing and care home staff
- Case managers and social workers
Patients can refer themselves, and, with their consent, a carer or relative can refer for them. When a patient or family referral is made, we will contact the patient directly and, with consent, talk to their GP and other healthcare professionals so that we can provide the best care possible.
How to Make a Referral?
The easiest way to refer is through the Strata system, available on most hospital and community networks. Third-sector referrals through COMPASS are also accepted.

Need Guidance?
If you’re not certain which hospice service would be most appropriate for a patient’s needs, we welcome an informal discussion before referral. We will review each referral on an individual basis in close discussion with patients and their families to determine the best support.
