Fast Track Service at St Mary’s Hospice
Our Fast-Track Service is designed to support individuals who choose to spend the final weeks of their lives in the comfort and familiarity of their own home. We understand how important it is for people to be surrounded by loved ones and familiar surroundings at such a significant time.
Our experienced and compassionate team of Healthcare Assistants provides tailored support, with up to four visits per day depending on individual needs. This can include help with personal care such as washing and dressing, assistance with meals, and support with taking medication.
We work closely with families, carers, and healthcare professionals to ensure care is responsive, respectful, and centred around the person’s wishes.
Referrals to the Fast-Track Service can be made by hospital teams, GPs, or district nurses. Families and carers can also contact us directly using our referral form.
Fast Track Service Eligibility
Who Is This Service For?
This service can be accessed for individuals who:
- Have chosen to remain at home to end-of-life care
- Have been clinically assessed by a health professional as being in their final weeks or days of life
- Have a life-limiting condition
- Reside in the Furness and South Lakes area
- Are 18 years and over
Service Description
- Support is provided for a maximum period of 12 weeks
- Care is provided seven days a week from 7.30am – 10.30pm
- Regular review ensures the service continues to be capable of meeting each patient’s evolving needs


Referral & Assessment Process
Once we receive a referral, the Hospice Care at Home Coordinator will arrange an initial assessment by an Integrated Team Sister who will establish a baseline of the patient’s condition.
Second and third follow-up assessments take place at weeks four and seven, respectively, with a multidisciplinary team review at week seven to determine ongoing eligibility for the service.
We want to provide palliative, high-quality care that respects every individual’s choices and assures dignity, comfort, and support at the end of life.
