Hospice at Home

Choice is essential at the end of life, and many people wish to spend their final days in the comfort of their own home. Our Hospice at Home service makes that possible, offering compassionate, specialist nursing care and support for you and your loved ones.

We are here to ensure you receive the care you need, where you feel most at peace. Serving the South Lakes, Furness, and Millom areas, we are dedicated to bringing comfort, dignity, and support to you and those who matter most.

We will help you choose the parts of care that best meet your needs such as:

  • Night sitting service
  • Personal care and comfort
  • Symptom management
  • Emotional, psychological and spiritual care
  • Practical support for those people important to you and who are caring for you

During difficult times, peace of mind at night can make all the difference. When available, our night sitting service offers comfort and reassurance by providing an experienced healthcare assistant to stay with you in your home overnight, from 10pm to 7am. Whether you need extra support or simply the reassurance of knowing someone is there, we are here to help.

How Can I Be Referred to Hospice at Home?

Your GP, Community Nurse, Specialist Nurse or other healthcare professional involved in your care can make a referral on your behalf.

However, you can get in touch with this service directly by calling 01229 580 305 or by visiting our how to make a referral page.

Once a referral has been received, a nurse will visit you and your family to discuss your needs. With your consent we will have a discussion with other health care professionals involved in your care. We will then develop and agree a plan with you to deliver the care and support you need. This plan will be reviewed on a regular basis.

All Hospice at Home staff carry a mobile phone, however, if there is no mobile signal they may need to use your landline. If you call us and we don’t answer, please leave a message and one of the team will call you back as soon as we can.

All Hospice at Home staff carry identification.

St Mary’s operates a no smoking policy. While we appreciate that we are a guest in your home, we would ask that you refrain from smoking while staff are present. If you have a night sitter, please ensure they have a separate room to sit in from anyone who is smoking. We thank you for your understanding.

What can the service do?

Our team is here to help you manage your symptoms and provide the support you need to return home from the hospital when that’s where you’d rather be or avoid a hospital stay all together.
We’ll be by your side, offering regular check-ins—whether in person at home or through a phone call—to ensure you feel supported. Working closely with your GP, Community or Specialist Nurse, and other healthcare professionals, we coordinate care that’s tailored to you, so you can focus on what matters most: comfort, peace, and time with loved ones.

In our team we have Registered Nurses and Health Care Assistants who are experienced in end of life care.

Who is the service for?

This service is for adults in the last weeks and days of their life, who wish to stay at home. This includes care homes and nursing homes.
