Making a referral is easy. Referrals to St Mary’s Hospice are normally received from your GP, Hospital Consultant, Specialist Nurse, District Nurse or Macmillan Nurse. You can even refer yourself. Please download the St Mary’s Hospice referral form via the following link:

Once referred your family carers also have access to a full range of services and support including In Patient Care, Hospice at Home, Family Support, and Living Well Services. So the sooner you get in touch the better! Thanks to the support and generosity of our community, all our services are free to anyone using them.
